TYPES OF SERVICES
☐ Repair in Office – Our technician comes directly to your office.
☐ Remote Support – Diagnosis and assistance performed via phone or online connection.
☐ Repair at US Dental Depot Facility – Equipment is shipped or delivered to our Boca Raton facility for service.

A Signed Conformity Release (checklist by service point) will be required after completion of services and before the technician leaves the premises.
*- Labor Cost: Charged at $140 per hour or any fraction thereof.
*- Travel Time: For distances over 40 miles from Coral Springs Florida, travel time is charged at $60 per hour.
*- Office Call Fee: The office call fee is essentially the cost of accessing the location, the professional time and expertise during an in-office visit $105
*- Minimum Hours Charge for Distant Locations: Locations far enough to require extended travel have a minimum daily charge of 6 hours.
*- Preparation Advice: It is advisable to compile a list of all your needs to facilitate anticipation of time and parts required.
*- Overnight Stay: If an overnight stay is necessary (due to the need for new parts, changes in job specifications, or additional services), a charge of $230 per night is applied for food and accommodations.
*- Initial Quote: An initial proforma invoice will be provided based on the information you supply.
*- Payment Terms: A credit card must be on file with pre-approval before services are rendered in other locations.
*- Additional Charges: Charges for certifications and plans (such as blueprints) are separate.
*- Conformity Release: A signed Conformity Release by checkpoint is required after service completion and before the technician departs from the premises.
*- DEVICES SENT FOR REPAIRING :
Diagnostic Charge: When a device is sent in for repair, a fixed fee ($100 in this case) is charged to diagnose the issue. This fee covers the labor and resources used to assess the device and identify what repairs are necessary. ( Sku 485025 )
Proposal: Based on the diagnosis, the repair service provides a proposal detailing the necessary repairs and their costs.
Credit Towards Repair: If the proposal is accepted and the customer agrees to proceed with the repairs, the initial diagnostic charge is then credited towards the total cost of the repair service. This means that the $100 paid initially is not an additional cost but is subtracted from the final bill for the repairs.
*- REMOTE SUPPORT :
Diagnostic & Estimate Repair – $140 (up to 1 hour, not longer).
Support by the Hour – $190 (or fraction thereof).
Support Package – 4 hours for $555. Invoiced as hours are used, by the hour or fractions larger than 10 minutes. ( Sku 485027 )
